Complete this form to request copies of any records pursuant to Wisconsin State Statutes 19.31-19.39 to include: Police Reports, Accident Reports, Citations, Pictures, Videos and other recordings. Once your request has been processed it will be emailed to you in digital or PDF form, unless requested to be delivered in hardcopy form. Additional fees and costs associated with your request may apply, including postage, shipping and handling and product fees for paper and physical copies.
