Ride Along Request Form

All citizens that request to ride along with an on duty Sauk Prairie Police Officer will be subject to a background check and approval by the Chief of Police or his/her designee prior to participating.

Please complete the Ride Along Request From below completely.  You may request to ride with a specific officer or list dates and times that work best for you.  An officer from the department will contact you to verify the approval or denial of the request as soon as possible. 

If approved, you must complete and sign a waiver of liability before participating in the ride along.
